For exotic animals, we ask that you provide any special diets as we only have a limited amount of exotic food for hospitalized patients.

I UNDERSTAND AND AGREE THAT IF ANY MEDICAL EMERGENCY DEVELOPS WITH MY PET(S) PAWS & CLAWS ANIMAL HOSPITAL WILL MAKE EVERY EFFORT TO CONTACT ME BEFORE ANY TREATMENT IS PREFORMED AND TO DISCUSS MY OPTIONS. IF I CANNOT BE REACHED I UNDERTAND THAT MY PET(S) WILL BE TREATED AS DEEMED BEST BY PAWS & CLAWS ANIMAL HOSPITAL, AT THEIR SOLE DISCRETION, AND THAT I WILL ASSUME FULL FINANCIAL RESPONSIBILITY FOR ANY AND ALL EXPENSES INVOLVED.

If you prefer your pets be boarded together in the same compartment (cat cage, dog cage or run), you're releasing Paws & Claws Animal Hospital from any liability that may result from that decision. You also understand that if a problem arises and the pets need to be boarded separately you will be charged accordingly.

**All animals must be free of external parasites (ex. ticks, fleas, etc.), or they will be treated at owner's expense.

**Paws & Claws Animal Hospital has my permission to do whatever is necessary should an emergency arise. **If a tranquilizer is necessary for treatment or handling, Paws & Claws Animal Hospital has my permission to administer such medication.
